The following documents are required at the time of enrollment.

Kindergarten

● Certified Birth Certificate

● Up to date Immunization Record

● Proof of Residency (most recent utility bill)

● Parent or Guardian ID

● Kindergarten Health Assessment (Kindergarten students only)

Kindergarten Health Assessments are available at Walker-Spivey Elementary School for you to take to your health care provider.

North Carolina law requires all Kindergarten students have proof of an up to date immunization record and a blue “North Carolina Kindergarten Health Assessment Report” form PPS-2K Rev 1/11 completed by a health care provider. The exam can not have been done more than 365 days before the first day of school. These are due upon enrollment. Your child will not be allowed to attend school if proof is not presented within the first 30 calendar days of enrollment.
